Após inserir no formulário a opção de foto aparece uma janela preta e mais nada. gostaria de tirar a minha foto para o formulário e site. Como?

I have taken a look at the only form you have in your account, but this form does not have any kind of File upload field or a picture taking widget.

If you wish for your form users to take a picture through the form and upload it with their form submission, you may want to take a look at our Take Photo widget:

The widget will run if you have a webcam on your computer, or if you open the form on a mobile device that has a camera. In the absence of a camera on that device where your users are viewing this form, the Take Photo widget may appear blank.

If you want your users to simply upload an existing picture from their computer, you may want to instead use one of the upload fields that we have available:
