My utility form is not sending the conditional emails

Good Morning,

Having an issue since last week on 11/9/16. Some of our leads aren't sending off to email on the conditions of the state they choose. If they choose. UT TX NM or CO the leads show up in our submissions but isn't emailing to the appropriate rep. Let me know if you see if these emails are being sent out not.
