    Only getting the first page of the form in the attachment

    On another point we have settings set to attach a PDF of the form submitted to the acknowledgement and notification emails but for some reason we only get the first page of the form in the attachment

    I cloned your form and I did a test submission. I received the PDF attachment but I wasn't able to replicate the issue. The PDF shows all 4 pages. Here's a screen capture:

    Please try to reset the PDF design of your submissions. Here's a guide:

    1. Select the form and then click the "Submissions" button.

    2. Click the "PDF" button and then select the "Customize PDF" option.

    3. You'll see the "Reset" button at the right.

    Ive followed your instructions and I can see the fornm in the body of the email but if I open the attached pdf all I get is the first page – see attached

    I think I may have found the issue.
    In viewing submissions, many of the fields are unselected in the box to the right of the top half of the screen. I select them all but cant see a save button so when I return they are all unselected. The pdf is actually showing the fields that are selected – how can I fix this?

    In order to save the changes you made in the properties of the submissions page, you need to click the icon again. Here's a screen capture:
