If I understood you correctly, you are unable to create Fillable PDF form from your form.
If the Fillable PDF form is not created in couple of minutes then it won't be created at all. Large forms can not be converted to Fillable PDF form. I have checked your form http://www.jotform.us/form/63194009405149 and it has lot of fields. I also saw that mentioned form has Form Collapse and spinner fields. Those fields can not be shown on Fillable PDF form.

Hi, thank you for the help. How can I get an offline version of this form? My client needs this for their customers and not wait 15 minutes for it to load. My client is a salesman for a liquor store and meets with restaurant and bar owners so an offline form of some sort would be great for them since they only have to load it once instead of loading every time they make a change.
Unfortunately I can only suggest you to create smaller form is you want to be able to create Fillable PDF form.
Your online form is also a bit slow because you have lot of fields (more that 800). I would suggest you to create separate form for each beverage.
Or maybe you can use different type of fields for example matrix field.

Thanks, I would love to split it into separate fields, but my client wants one massive list so he can have one single form for his customers to look at.

You can also use Purchase Order payment field, but please note that this field will not be show inside of Fillable PDf form.
