I have checked your form and sent a test submissions to your form. All submissions sent successfully as you can see below:
The submission that have number #51403393201846 is my submission that I filled form and submit. Other two mails are sent after click "Test Mail" button.

We are sorry for the misunderstanding.
Notification e-mail has been sending to the firstname.lastname@example.org instead of to email@example.com just because of the fact that the recipient e-mail on notification setting is set to firstname.lastname@example.org.
If you would like notification e-mail is received by email@example.com, you need to change recipient e-mail.
