the form. Originally, I had the form embedded in my WordPress site and Wix
site. I removed it from those sites. The embed still doesn't work.

I checked your website page and indeed the form was not embedded anymore. I do see the button/link only of the Registration form.
Please try also re-embedding your form https://www.jotform.us/form/63275774227160 again on your website using instead its iframe publish code.

You can try again the form when published using its iframe code.
