Uploaded files are not shown as attachments inside of Google Drive

I have a form with a few attachment fields, where each allow multiple attachments.

Lately most of the Google Drive folders are not created with all attachments. I havent't seen a pattern, sometimes pdf files are not inserted, sometimes jpg files. Is there a problem?

I tested our Google Drive integration several times, and I wasn't able to replicate this issue, uploads were going through with PDF submissions and files attached. 

I see that you have 9 submissions total for your form, could you please let us know which submission's files you have missing in Google Drive so we can try to compare and find the cause of this issue?

For instance today there was 1 submission at 2016-11-25 07:27:16. The filled form appeared on its folder in google drive, but none of the attachments were included as separate files.

Submission that you provided has 5 uploaded files 4 .jpg and 1 .pdf.

I have also made few test and I was not able to replicate mentioned issue. All files that I uploaded were shown inside of my Google Drive.

Can you please try to re-integrate your form with Google Drive to see if that will resolve your issue. Remove current integration and then integrate your form again with Google Drive.
