Who files Form 5498?
The trustee or issuer of the IRA files it, not the account holder. That's usually a bank, brokerage, credit union, or insurance company. If you're the account holder, you'll get a copy for your records, but you don't attach it to your tax return.
Why does Form 5498 arrive after tax season?
You can make IRA contributions for a given year up until the tax filing deadline the following spring, so trustees have until May 31 to file Form 5498 and send it to account holders. A separate statement of the account's fair market value is generally due to the account holder by January 31.
How do I fill out Form 5498?
Enter the trustee's or issuer's name, address, and TIN, then the participant's name, TIN, and address, plus an account number if you use one. Next, fill in the boxes that apply: regular IRA contributions in box 1, rollovers in box 2, Roth conversions in box 3, recharacterized contributions in box 4, and the fair market value of the account in box 5.
