Can I have a template form that I add different information then send to different clients?

    I work in construction and I would like to create a form that allows us to bill clients for changes that they are requesting. Obviously, every project is different and every change is different from paint to drywall to cleaning.

    Ultimately, I would like to have a template that we use across our company where we can take a picture of each change with a unique description that I can fill out, then send it to them and sign for approval.

    I see you form, it's a good start to accomplish the form that you'd like to have. Maybe, you should add a textbox for your clients to enter what type of service that they needed or a dropdown with a list of services that you offer. 

    If you'll be using a dropdown, you can assign the options a calculation value. With this, you can get the total for your clients to approve.

    For the approval, I would suggest adding an email field where you can put your client's email address. Add an autoresponder email (https://www.jotform.com/help/26-Creating-a-Form-Autoresponder) and use this email field as the recipient. You can customize the message inside the autoresponder.

    For your clients to approve the service and its price, you need to add the edit link in the autoresponder email body.

    You already have a checkbox with an approval options to choose from. But instead of this type of field, use the Radio button for single selection. I would suggest adding a new field where your clients can enter their message for this approval.
