    Hello Maicon,

    Thanks for reaching out to Jotform Support. Unfortunately, our Portuguese Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.

    Now, let me help you with your question. Please note that the Configurable List widget cannot be configured to have a “Take photo” option added to it. I recommend that you try including a textbox where users can input the URL of their images. Following that, you can suggest that they upload their images to a third-party application such as Dropbox or Google Drive, get the public URL, and paste it into the textbox you added to your Configurable list. Let me show you how:

    • In Form Builder, click on the Add Form Element menu on the left side of the page.
    • Under the Widgets tab, add the Configurable List widget into your form.
    • You can customize the fields using the Fields Configuration Box. In the box, use the syntax below (click here to learn more).
    {label} : {type} : {placeholder}
    • After finishing all the customizations, click on Update Widget, and you're done.

    Also, we have a File Upload element that makes it easy for anyone to attach images or a Take Photo widget that capture images real time when they fill out a form.

    Give it a try and reach out again if you have any other questions.
