How do I import a template from Word or PDF to JotForm?

I have a PDF form that I want to convert into a Fillable PDF to put on my website, however, JotForm is not allowing me to import it. It says "Please Select" but it does not let me select anything except "Please Select".

Unfortunately, the tool we have to import a form from a PDF file will work only with Adobe Forms Central forms, otherwise your PDF file will not work.
