You can import submissions data from one form to another by following this guide https://www.jotform.com/help/308-Import-App-Easily-import-your-Excel-or-CSV-data-into-JotForm.
So basically, you will have to export your form submissions data from the old form to excel file.
Modify the excel file with the headers provided by the app and then import it into the new form.
