My form is wrong.
How can I solve this problem?

That message is shown when Unique Submission or Unique Question are set inside of the form to prevent multiple submissions by same user.
If you want to allow your users to submit your form multiple times, than you show set this options to No Check.
If you still have issues, please provide us ID of your form.
