How to use these forms in my website ?

First of all a big Thank you to you. You are a savior.
But I am having this real problem, I am not able to use these forms on my site.
While clicking on publish I am getting various options and I have tried all of them, but when clicking on preview changes I am not getting anything. 

Please Help. 

If you are trying to add the form to your web page, you would copy your forms embed code and paste it in with your web pages' code:

Depending on what type of website builder you are using, some embed code may work better than others.  The iframe code tends to work best:

If you are simply trying to share a link to the form, you can simply send the URL of your form to your users via email or add a hyperlink to your site:
