Parents have contacted me because they are not allowed to submit a second form after the first. I cannot find how to allow it.

Do you have issues with 2017 Club Membership Registration Form? That is the only form on your account.
Error message that you provided is shown when Unique Submissions feature or Unique Question feature is enabled on the form. However I have inspected mentioned form and both options are set to No Check.
I tested mentioned form two times and form was submitted successfully both times, so it seems that you were able to resolve your issue.
If you have issues with some other form, make sure that Unique Submissions and Unique Question options are set to No Check, like on screenshot above.
If the issue still persist on your end, please provide us ID of the form or URL of the webpage if the form is embedded.
