I have a form with dozens of conditions. Today when I was making some changes and testing it out I realized that probably 100 condition were simply gone. I did not remove them. What could have caused this?

I'm sorry for the trouble caused to you.
I did check your form's revision history and found that earlier today, your form was edited and conditions were removed. Please check the screenshot below:
Since it shows 197 changes together, I believe there is some problem. Can you please ensure that you have not logged in from two different browser and loaded the form in editor. Please note that auto-save feature is enabled in form builder and if you have opened the form in two different browser, one version will always overwrite the version of another.
For now, I would suggest you to please take advantage of our "Revision History" feature to revert back your form to an earlier working version. The following guide should help you: https://www.jotform.com/help/294-How-to-View-Form-Revision-History

Thanks for the response. It may have been from logging in with a different browser but I do not recall doing that. Hopefully it does not happen again!

I would suggest you to check your account activity log in order to know where you logged from: https://www.jotform.com/help/370-How-to-View-Your-Account-Activity-Logs
However, if you notice your conditions get deleted again and you did not do this action, please let us know, we will be glad to help you.
