Hello,
I have a form that allows a donor to purchase a certificate of appreciation. I want to offer other options. The form would be identical except there would be three options - one for up to 3 certificates (currently offered) one for up to 6 certificates and one for up to 10 certificates. Do I have to create a form for each option OR is there a way to have 3 buttons for the viewer to select from and have what they see and pay for correspond to that button?
Thanks for any insight...
Nancy

I assume that your question is related to "Purchase" field on the form you provided.
You can update your form and add other options. You can then use hide show conditional logic to show "Enter Information" fields, base on option that was selected. For example if up to 6 certificates option is selected, only 6 "Enter Information" fields will be shown.
More about hide/show conditional logic you can find inside of this guides:
Show or Hide Multiple Fields at Once Using the Form Collapse Tool
How to Show or Hide Fields Base on User's Answer
If you will add payment field to your form, you can perform calculation with your "Purchase" field and pass calculation to payment field of your form.

Hope this will help. Let us know if you need further assistance.

I will try this.
Next problem. I changed the title of my form but when I try to share the form from the jotform tool to Facebook it continues to show the old name. I can't seem to find anywhere else that has that name but it keeps showing up. I have tried signing out and signing back in, I have also tried restarting my computer. Neither action fixed it. -

Considering that you issue with the form title is not related to subject of this thread, I have moved your question to separate thread and we will provide you an answer there shortly.

We have to move new questions or issues to separate threads, to avoid possible confusion if you have any followup questions.

Since figuring this out on my own as a non-web developer is proving to be impossible and without any real-time support I could waste yet another entire day trying to make it work without success is there any way to PAY jotform to create the form I am looking for?

I was able to get the 3 choices for payment types and the ten options I wanted available. I am still not understanding how to limit each of the three choices for payment for the appropriate number of options using the conditional function. I want choice one to show options, choice 2 to show six and choice 3 to show all 10.

You can add the options to each of the item in your payment field.
Ex:

Let us know if you are still not able to do it.
