I cannot login as it says I am suspended

    Your account (jaredbradysealcom) is currently suspended. I checked your form and I can see that you have fields that are asking for bank information and SSN.

    As per our terms of use, collecting sensitive information and phishing forms are not allowed here JotForm. 

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    We need to remove these fields so that we can re-activate your account. If that is okay with you, please let us know.

     

    If you really need to collect this information from the user, then you need to add the following into the form:

    1. An agreement stating the use of bank details. You can use the "Text" tool. You may also use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. Digital signature field. You can use the E-Signature widget. Here's a guide: How-to-Add-an-E-Signature-to-Your-Form.

    3. Use form encryption. Here's a guide: Encrypted-Forms-and-How-to-Use-Them(Optional).

    Yes I apologize for lack of research on my part you can remove the form and
    I will use a different way to collect the info,.
