Jot form is not sending the completed form

    When I get the forms from clients. They are cut off to almost half of the information the client filled 

    out. When they are placed in evernote.

    Hello deeprootsclinic,

    I'm sorry for the trouble caused to you. I did check your form's notification email alert and it seems you have edited it. Your form's email alert template does not display all the questions.

    Please note that if you edit your email alert, it becomes dirty. That mean if you make any changes in your form after editing your email alert, the changes will not reflect in email template. The best way to solve this problem is to delete your existing email alert and add a new one. A new email alert will always include all your questions of form. I would suggest you to please delete your email alert and add a new one. The following guide should help you:
