You wanted this to be passed from one form to another correct? This is achievable but there's a requirement:
1. The Input Table should use the same set of fields if it will receive data from a different from
If Form 1 has Single Choice (radio button) Fields, then the Input Table on Form 2 should use Radio Button fields.
Same applies if you're using a Multiple Choice (checkbox) fields. The receiving Input Table on Form 2 should also use Checkboxes.
2. The options of the Single/Multiple Choice fields should correspond with the Column Names of the Input Table.
