Heading image does not appear on PDF

I have created a page within a form with three headings, each one with a title, sub-heading and an image. When I save the submission pdf the images disappear, is there a way to make them visible?

Unfortunately, the header image will not appear in the PDF submission. I suggest you use the "Text" tool and then enable the "Show Headers and Texts" on the Submissions page. Here's a guide: How-to-Show-Headers-and-Text-in-the-PDF.
