How to create an online fillable genealogy form?

I'm not clear on what you're offering.  What I'm looking for is the ability to create a form on my computer that I can email to multiple relatives, that they can fill out on-line, print out locally if they want to, and email back to me or to others.  Do you offer this?  How do I get started with it?  How do I design a form?  What does it cost?        Paul Meyer
