How can I import information from a spreadsheet?

I want to be able to enter some basic information......   names addresses etc and have them auto populate a suite of forms all regarding one person....  name, doctor, birthdate etc...

As I can understand, you want to have some data to pre-populate your form. Please note that this can be done, but importing the data from a spreadsheet.

You have to have an Excel sheet that will have the same fields like in your form. Also, the widget will work only with these fields:

So address field will not work.

What are the fields you want to prefill from the Excel sheet? And I assume you're working on this form: http://www.jotform.us/form/72648447984171?

I know the address field is there now. But mostly i was using short texts. Is the field name the question?
