How to create an Employee Feedback survey?

    We would like to send out an employee survey where everyone can leave feedback on each other, but would like a means to put identification on each employee. So when that exact employee gets their survey, their own name won't show up. Is that possible?

    Hi Josh,

    Thanks for reaching out to Jotform Support. Just to clarify, you would like your form users to fill up the form with a Name field but do not want the value from the Name field to be sent out to the form user via email. Is that correct? If yes, you can update the Autoresponder email by removing the Name field, which is the email that is sent out based on the email addresses provided in the Email field. Here's how to do it:

    1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.

    2. Click on Emails on the left side of the screen.

    3. Hover your cursor over the Autoresponder email and click on the Pencil icon.

    4. In Email Content, delete the row for Name.

    5. Click on Save.

    Not quite, what I'm looking for is that if I have a team of person a,b and c. If I send this survey to person a they would be rating person b and c. On the contrary, person b would only see a and c when filling out the survey. We do not want people to see their own name when rating.

    Hi Josh,

    Thanks for your explanation. You can try adding a question field in your form with the list of their names. Then, create a Show and Hide conditions to Show the questions depending on the answer to that. I'll walk you through setting that up:

    1. In Form Builder, click on the Add Form Element menu and add a Dropdown field.
    2. Go to the Options tab and add the names of the participants.
    3. Then, add your feedback questions and Hide them.
    4. Finally, create Show and Hide conditions to only show the questions based on the name selected.
