Why is there missing information on my email notifications?

In het overzicht staat alleen :

ID (nummer), Datum Inzending ,IP (nummer)

I have been checking your form and it seems like the submissions show the correct data, but the problem is with the emails, I would recommend you to delete the emails and add them again, this should have a fresh copy of your fields:

Also, note that most of your fields are not required so they may be sent empty, if you need those fields filled then you need to require them from the settings as well.
