Thank You Page/Card Form: Show different Thank You Page based on the answer

    The first question on our survey is "Do you have a child between the ages of 0 - 6? YES or NO. If they answer YES they continue the survey, if they answer NO then I want the survey to give them a thank you page and complete the survey. On our Thank You page right now; we have a link to a separate JotForm to enter their email address into a lottery for gift cards. However, I don't want the people who answer NO to be capable of entering into the lottery because they didn't give us any data. The reason we have the separate link for the email addresses is to keep the original survey anonymous and not tie an email address to anyone's survey submissions.

    Hi jmccully,

    Thanks for reaching out to Jotform Support. Card Form layout has limited customization. The Action Button on the main Thank You Page(second form), will remain despite adding a Change Thank You Page condition. I suggest switching to the Classic Form layout to conditionally show a different Thank You Page. However, you have several Yes/No Question elements which is not available in the Classic Form layout. You need to change it into the Single Choice field. Then add a Page Break element on each question to look like a Card form. It's gonna be like creating the form from scratch again. But let us know if you want us to walk you through that.

    However, the other option is to remove the Action Button button on the Thank You Page, add an Email field in your form, and link the second form to the Autoresponder email. Here's an example:

    1. In Form Builder, click the Action Button at the bottom of the form and click the Trash icon to delete. Then just edit the text in the Thank You Page with a message that they will receive an email with a second form.     3. Then create a Show/Hide Field condition to hide the Email field for those who selected No. Thank You Page/Card Form: Show different Thank You Page based on the answer Image 3 Screenshot 82
    4. Then click on Settings at the top.
    5. In the menu on the left side of the page, click on Emails.
    6. Now, click on Add an Email.
    7. Next, select Autoresponder Email.

    Hi there! Thank you for the quick response! I have gone through and created the form in the classic setting instead of card setting, but I'm still having trouble doing the different Thank You pages, one with a link to other form and one without that link.


    It would be great if I could sit down with someone on your team and have them walk me through it.

    Thanks for getting back to us. When I cloned and tested your form, the same thing happened to me. After investigating it a bit, I found that there's one Change Thank You Page condition that causing it. Please delete the condition shown in the screenshot below:

    It worked, thank you so much! I just need to finalize the A, B, C conditions and then the survey will be complete.
