I checked your form on an iPhone 8 and could not reproduce the problem:
It looks fine:
We will need more information:
1) What iPhone version is affected?
2) What web browser is affected?
3) Can you post a screenshot of the problem?
How-to-Post-Screenshots-to-Our-Support-Forum
4) Can you tell us exactly how to reproduce the problem?

I checked your form on an iPhone 6. And the signature field is working for me:
One thing you can try is to use a different signature widget.
You are using the smooth signature widget.
Please try the E-signature widget instead:

All you have to do is search on the left under the widgets tab for "E-Signature".
Then drag and drop the widget onto your form.
