HIPAA: Form is disabled due to HIPAA.

    It appears My jotform has been disabled because of a Hippa compliance upgrade. However I am not receiving information from my clients that requires me to upgrade. Can you assist in reactivating my jotform please?

    Hello kokofitness88,

    Thanks for reaching out to Jotform Support. If you are not taking any Personal health information, you need to select NO, I'm not a HIPAA-covered entity option in your Account Settings. Let me show you how:

    1. Hover over your Profile/Avatar icon.
    2. Click on Settings and go to Data in the menu on the left.
    3. Once you open the Data page, you will see HIPAA initiation modal.

    4. Now, select “NO, I'm not a HIPAA-covered entity” option.

    After selecting NO, I'm not a HIPAA-covered entity, you need to enable your form. Enabling your form is easy. Let me show you how to do that:

    1. In Form Builder, go to Settings in the orange navigation bar at the top.
    2. Click on Form Settings and set your Form status to Enable. And click on Save.

    Thank you for your response. Unfortunately I am not able to make that
    change. My form is greyed out, I cannot select it and enable it. See
    attached screenshot.. Is there anything you can do on your end?

    Thanks for reaching out to Jotform Support. May I know, did you select NO, I'm not a HIPAA-covered entity? Unfortunately, your screenshot didn't go through correctly and, we're unable to see it. Can you please try to attach it again? This guide will show you how to do that.

    You're being asked to upgrade to HIPAA because our system detects that you're dealing with Personal Health Information. As required by laws within the US. But if you're not HIPAA covered entity, allow me some time to coordinate this with the relevant team and we’ll get back to you as soon as we have an update.

    Hi there,

    We've enabled the form. When you log into your account, you'll see a HIPAA pop up message, click on the Go Back link on that message and then click "No, I'm not a HIPAA covered entity". That's it. You should no longer receive the prompts or disabling of forms again.
