Jotform App : Medical

Hello Team,

I have some question about apps that i am going to use..

First of all can i able to make apps direct i can collect data from forms and first user will registered in our apps then they are able to fill up the other forms like medical history collect form and i need after the fill up the the admin or doctor can collect the information and doctor will able to send notes to users or example medicine they need to take so user login in this panel the see the doctor notes please.

Also this app like i need to upload on app or play store if yes your team will do that or we need to also they are accessible from direct url if yes i need to connect this with my domain only.

Can you please answer so i can process further.... i see below urls

Jotform App is an online form builder that lets you create forms for data collection even when you are offline. This app will let you create your forms, please note that you can use it to create portals to your sites, documents, and forms for easy access. Furthermore, please be inform that collection submission data will stored in your Jotform account. You can share it with your admin or doctor by Collaboration, but the other person must need to have Jotform account so that they can able to access it.

Please check below demo that i see i need all 8 points that video describe there is any predefined demo available it will be really appreciate.

Thanks for getting back to us. To achieve the 7 points stated in the video you shared, you can add Apps elements and widgets. You can add the appointment field to your Jotform form which you can add to your app to let your patients book appointments through the app. There are also several other apps elements and widgets to use for the other functionalities you need on your app. Check out the screencast below to see how the app builder works:

Thanks for getting back to us. Unfortunately, what you are trying to implement is not possible as users won't have to register to the app. But, you can set the app's privacy settings to Private, so they'll be asked to create a Jotform account and log-in to their Jotform Account every time they access your app. The Streamlined Patient Onboarding point on the demo you provided above, means letting your App Users create a Jotform account so they'll be able to maximize the app as well as to secure your App by not letting anyone access it.
