    Good Afternoon,

Can you tell me how I can add more information to the PDF submission?

I want it to display more of a description of the selection as per this form

    https://form.jotform.co/73478472764874

When the medication required is selected eg. Lignocaine topical anaesthetic there is a description that is displayed.

When the PDF is generated, this description is not displaying. Is there a way I can make that happen?

What you're describing are the TEXT FIELDS on your form. These fields are not included on the PDF submission by default. You can enable them by going to your SUBMISSIONS PAGE > click the the small GEAR icon > then tick the SHOW HEADERS AND TEXTS checkbox.

However, these won't conditionally work the same way you have set them up on your form. Following the steps above will only cause all your text fields to appear on your form's pdf submission.

Alternatively, you can also manually edit your PDF template upon your discretion and add/remove fields that you wanted to be included/excluded respectively.
