My form keeps disabling on its own. I have changed back to Enable multiple times. Please advise.

Hi Morgan.
Thanks for reaching out to Jotform Support. I've checked your account and see that some of your forms have been disabled because we see that they're collecting protected health information (PHI) but your account is not HIPAA-compliant. To keep this information secure according to HIPAA requirements, you must enable HIPAA compliance features for your account before you can continue using these forms.
The HIPAA compliance features are available on the Gold plan. You'll need to upgrade to this plan before you can enable those features. It's really easy to upgrade your account. Let me show you how:
1. On your My Forms page, click on Pricing in the blue navigation bar at the top of the page.
You can also upgrade your subscription on your account’s Upgrade/Billing page.
2. On the Pricing page, choose the plan you want to buy.
3. In the popup window, choose either a Monthly or Yearly payment schedule and either Credit/Debit Card or PayPal as the payment method.
Here’s an example of the Bronze plan:
After you click on Continue, you'll need to enter your Credit/Debit Card details.
If you choose to use PayPal, you’ll get redirected to the PayPal payment page to complete the payment. Check out this guide for more details about upgrading your account.

they do not fall under HIPAA regulations. Please consider this further.

Thanks for getting back to us. May I know if you received an email stating that you may be a HIPAA-covered entity? If so, if you are not a covered entity under HIPAA, kindly respond to the email by stating that you are not. This way, your forms are not going to be disabled. We need to ensure that the users are not collecting sensitive health information without being covered.
Additionally, if you have been prompted by the message below when you log into your account or on your Account Data page, please click No so that you will not be identified as a HIPAA-covered entity.
If not, please log out from your account and log back in then and click Go Back when the message pops up.
Click on NO, I'm not a HIPAA-covered entity.
Give it a try and let us know how it goes.
