    Hello ehtravels,

    If what you would like to do is to change the part of the URL such as the form ID, you may do so. This guide can give you more information on how to make it happen: https://www.jotform.com/help/261-How-to-Create-a-Custom-URL-of-a-Form

    If what you want is to change the actual domain of the form, such as using your own domain in place for the form, please check this guide: https://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform

    On the other hand, you can easily embed forms to your web pages, with this you do not need to change the URL of your form. By embedding, the form will appear on the page where you have placed it.
