Simple one page form

Until we take more time to understand how to create a more detailed form we could use some help to put together a simple one page order form so that our community can have the option of ordering 1 item at a set price. Name,email, phone, brief description of what they are getting and option to use Square. I see that there are alot of options but we only need the 1 order form until the season gets started.  If you have a step by step fast track way for us to accomplish this it would be greatly appreciated.  Thanks, Mark
