Is is possible on Jotform that same survey can be filled out multiple times by same people?

Hi Jotform,

I just stumbled across your form and would like to know if your system can handle the following situation.

I need one form that I need to send out to 10 managers. Each manager owns 6 facilities. Ultimately I need each manager to fill out the form 6 times - one for each facility.

I would like each form to be organizable or trackable by the manager and the site name.

I also need to add some photos and images to the form.

I would also like to be able to have a way to easily identify (for me) after tallying the results the sites are the compliant (say, get a green check) and non compliant sites.

Can your system easily do this?

Thanks for reaching out to Jotform Support. Yes, it is possible to create a form which can be filled out multiple times by the same people. You can explore the manual prefill option and survey form. You can create survey forms at Jotform. We have a number of useful tools and Templates Customized for Surveys. We also have some Getting Started with Jotform guides to help you.

Here are the steps on how to use the Manula prefill option:

1. Open your form in Form Builder, then click on Publish in the orange navigation bar at the top of the screen.
2. Now, click on Prefill in the menu on the left side of the screen, and then select Manual Prefill. 
3. Click on the + Add a New Prefill option. Then fill out the basic details for the Manager you wish to assign your form. 
4. Choose Send Invitation option to send the invitation or share the prefill URL link, that's it.

You can add the Form Calculator widget to calculate the score of the questions and then use the conditional logic to show if your site is compliant or non-compliant using the Change Thank you, Page. 

To add the image field, you can use the File upload element to get the images uploaded by your form fillers.
