Form submission view is different compared to the live form

Hi in ma page : 

the design of my jot form is not good, it's not the same than in my dashboard.....

And somes questions ard skiped......  see only the header section, not the forme between...

Can you please review again the submissions in your form View Submissions panel.

The Headers and Text fields should also show in the submissions now.
