We need a code to attach to a radio button on our website

Were gearing up to go live with our "50 For 50" Campaign and when I set up the Square account to receive and deposit the donations, they told me I needed a code from Jotforms to attach to the donate button on our website so that Square would receive the payments.

Please let me know the process of getting this done as soon as possible.
