Form template that directs user to a different form based on the answer from the first form

    do you have a template/form that asks something like this..

    1) How many adults are in your household?

    2) How many children under 18 are in your house hold?

    If they respond 1 adult : 0 Child - it takes you to a specific form to complete

    If they respond 2 adult : 0 Child - it takes you to a different yet specific form to complete

    If they respond 2 adult : 1 Child - it takes you to yet another different specific form to complete?

    and so on...

    Hi Theresa,

    Thanks for reaching out to Jotform Support. We don't have a form template same as your requirement. But that's possible by adding a Change Thank You Page condition to direct your user to a different form based on their answer to a question.

    1. In Form Builder, add your fields. You can use a Dropdown element or a Short Text element. In this example, I added a Dropdown element. Form template that directs user to a different form based on the answer from the first form Image 1 Screenshot 50
    2. Next, add a Change Thank You Page condition. Click on Settings at the top.
    3. Select Conditions on the left side.

    5. Create the condition like below:
    • IF: (Question)
    • STATE: Is Equal To
    • VALUE: (Answer to the question)
    • DO: Redirect to URL

    You need to create multiple Change Thank You page conditions to redirect your user to a different form. You can also create one form and conditionally show the other fields based on the previous answer because based on your requirements, you need to create multiple forms from scratch. I suggest adding all fields in one form and conditionally showing the other fields by adding a Show/Hide Field condition.
