I need to create a form to keep a log of incoming calls from residents and vendors

I have received call from residents and vendors and I need to capture their contact information and what the issue is.  Then forward that information to a Regional Property Manager and Property Manager.  And, be able to generate a report from these incoming calls.  What is the best template to use?
