How to Build Test Forms

Hi! i need someone to create a system that calculates the appropriate pay rate based on the written test grade, hands-on test results (correct or incorrect), and the time taken to complete the hands-on test, we'll need to establish some parameters and criteria for each component: Written Test Grade: This could be on a scale of, for example, 0 to 100. Hands-On Test Results: We need to define how you'll assess correctness. Will it be a binary correct/incorrect assessment, or will there be levels of correctness? Also, the time taken to complete the hands-on test is essential. Pay Rate Calculation: Based on the written test grade, hands-on test results, and time taken, you'll need a formula or set of rules to determine the appropriate pay rate. Here's a basic outline of how you might structure the calculation: Written Test Grade Weighting: Assign a weight to the written test grade. For example, you might decide it's 40% of the total score. Hands-On Test Results Weighting: Assign a weight to the hands-on test results. This could be split between correctness and time taken. For example, correctness could be 40% and time taken 20%. Pay Rate Assignment: Based on the combined score from the written test and hands-on test, you can assign pay rates according to predetermined criteria. This could be in the form of a lookup table or a formula.

So we have 5 test test, Math, tape, tool recognition, color recognition, and a journeyman electrician test. I have these test, this test also needs to be built into jotform.

    its possible in jotform

Below is a breakdown of what I need. Please let me know if you have any questions and if you will be able to complete this project.

I am looking for a form that will out put data based on test scores and time spent to take the test.

The test consists of Two Sections

    -       Written Test

    -       Hands On Test

Written Test consists of the following below. Each of these test will need to be created as well. I can provide the questions and correct answers for each test. Based on the score of each test, it will be used to be a factor in the overall score and a candidates payrate.

    -       Math Test

    -       Tool Recognintion

    -       Color Recognition

    -       Tape Measurement

    -       Journeyman Test

The Hands On Test will consist of the following below, this information I will also be able to provide. The weighted factors will be broken down into 4 sections. Each section is timed.

    -       E1

o  This section will have 3 questions

    -       E2

o  This section will have 3 questions

    -       E3

o  This section will have 3 questions

    -       Journeyman

o  This section will have 5 sections

Candidate Level: This test will also need to be weighted based on if the candidate is one of the following, the more experience a candidate has the more their test is weighted. The candidate level is the foundation on how the results are calculated. The less experience the less it should be weighted.

    -       E1

    -       E2

    -       E3

    -       Journeyman

In the end I am looking for all the appropriate data/formulas and test to be created so It can output the proper payrate.

I want a candidate to come into our office, take the written test, those grades are to auto populate and provide a grade. Once they have finished the written test, I need a form asking the hands on test questions.

    For example

Did candidate insert pipe correctly? Yes or no?

Did candidate place material safely? Yes or no?

How long did it take candidate to complete this section? (INSERT Number of minutes)

Once all fields are completed. I need a submit button that will then take the following below into consideration and calculate a payrate based on the candidates experience level.

    -       Candidate experience level

    -       Written test

    -       Hands on test

    -       Output payrate

I hope this makes sense. Please let me know if you have any questions.

    Hello Farzana,

Thanks for patiently waiting. Let me show you how to achieve your form request:

    • To see the exact time for each form submission, you can use the Time Tracker Widget. It will show you how much time a user spent on your form in seconds, minutes, or hours.

    • To create a graded test, we have an available feature called Quizform that can be used to grade each question. Let me show you how:
    1. Go to Quizform, then select and click Open Quiz Form option at the bottom.
    2. Click Allow in the authorization dialogue box that will appear.

    3. Once the Form Picker dialogue box appears, choose your form and click Continue in the lower-right corner.
Choose which of the options you want to be selected. I suggest enabling the Show Results to user after submission.
