How can I reactivate my suspended account?

Good Afternoon,

I got a message that my account was suspended due to phishing with a form I created called Liquor Application.  I realize I was asking for SSN (I am an attorney and need this information for application for liquor licenses), I apologize for that and can delete that question from the form immediately if my account is reinstated.  Please let me know what is needed to reinstate my account.

Your form was flagged as phishing since SSN number is considered as sensitive information, please note that in order to reactivate your account I had to remove such field. 

Now, if your purpose is to collect such information from your customers, then you may add the field again only if you add some sort of agreement and a signature box, this way the users know and agree to provide their information.
