Auto-responder email alert is not being displayed correctly.

    Hello,

    I continue to receive concerns from folks who complete our form and report that the column information is mixed up.

    Most recent message I received:

    "I just "attended" a meeting this afternoon - it was from this weeks program yet I still get a form coming  back to me without the boxes filled in correctly (even though I inputted the information in the correct box - ie I had Edmonton down as the Club name and 5370 as the District but the form didn't translate it that way) and some I don't recall being there for me to fill out (ie "How much time did you spend on our website")."

    I am not sure if I have understood your question correctly. 

    Do you mean to say that the auto-responder email which is sent to user is not displaying the responses correctly and also it displays some non-existing question?

    It seems your notification email alert template is edited. I would suggest you to please delete your existing auto-responder email alert and add a new one. Please note that a new auto-responder email alert will always include all form fields of form. The following guide should help you:
