Creating my first order form

I need to create an order form to be able to:

-offer multiple products

-with multiple choices within each product

-with PHOTOS for each product shown   Is there a template to do this?

Also for this:

-Ineed to be able to use SQUARE (not paypal).

The one you had suggested for this only allows paypal.

Please help and respond!  I would like to try JotForm vs Google form.
