How to upload my waiver form?

    I am looking to make waivers and a registration form to use on a static ipad at our rental location and store the waivers in a searchable base.

    Hi Ken,

    Thanks for getting back to us. Yes, you have the option to access your Jotform form on an iPad. This can be achieved through two methods. Firstly, you can launch a browser on your iPad and navigate to your Jotform form. Alternatively, you can utilize the Jotform Mobile Form App, which can be installed on your iPad via the following link.

    Additionally, you might also be interested on using your Jotform form in a Kiosk Mode. We have a guide about How to Use Kiosk Mode in Jotform Mobile Forms that you can check out.

    Is it easy for me to upload my waiver? And can I have two forms instead of just one and Ken one of the former have a courier bowl question and answer a quiz design to work on the website?

    Hi Ken,

    Thanks for getting back to us. Yes, you can create up to 5 forms for the Starter plan. As for the waiver that you want to upload, you can upload it as a  Smart PDF form. Let me show you how:

    1. Go to the My Forms page and click the Create Form button.I need help setting up my forms Screenshot 60 Screenshot 20

    3. Select the Import PDF Form option. Clicking this option will take you to the upload page.I need help setting up my forms Screenshot 82 Screenshot 42
    4. On the Upload tab of the Smart PDF Forms builder, you can drag and drop your PDF or click the Upload PDF Form button to select your file. Upload the file to proceed.I need help setting up my forms Screenshot 93 Screenshot 53

    After the upload is complete, Jotform Smart PDF Forms will take you to the Build Step, where you’ll see the online form (on the left) and the original PDF (on the right) side by side. In the Smart PDF Forms builder, you can adjust your online form and the PDF, such as connecting fields, adding missing and new fields, deleting fields, etc. Check out the GIF image below to see how it works:
