Need some help to make a form that does forumlas

Can you please help me...

I want to work out commissions. The spaced that are grey are forumulas from Excel and don't need editing but i want to turn this into a form.. With the 2,800,000 being the only thing that is an input and the rest is calculated at a formula.

The 2500 needs to be an editable space as well as the number of agents.
