Best Questionnaire Template to Use?

    Hello, I am needing an interview questionnaire form.

    Basically there are 500 questions, but based on the category you select in the beginning, determines which 100 question you are then asked, and then from those 100 question the interviewee selects 10 for them to answer. I also need it to have the capability to updated photos, around 5, 1 portrait, 1 logo, and 3 of their work.

    Maybe even a login passcode feature for the interviewee?
    Then have it linked through my squarespace.


    Is this possible? If so what is the best template to start off with?

    Hi Alex Hart,

    Thanks for reaching out to Jotform Support. You might want to take a look at these Questionnaire Templates to get an idea of the form you'll be creating. You can use the forms, test them, and customize them to suit your needs. You can also create a form from scratch and select the fields and widgets you will need. 

    For the category, you can use the Section Collapse element and add the 100 questions to each of the 5 Section Collapses. Let me show you how:

    1. In Form Builder, select Add Form Element in the upper-left corner.
    2. Scroll down to Page Elements and select Section Collapse.

    I’m looking for something different. I am going to be interviewing people in different industries. So my first question would be what they do?     Hi Alex Hart,

    Thanks for getting back to us. The Questionnaire Templates that we currently have are ready to use so that you won't need to create a form from scratch. However, if you want to have a questionnaire that has a specific question and will show a set of questions based on the answer in the previous field, you will have to manually edit these templates according to what you want.

    You can also hire some of our Jotform experts on this page who will help you build your forms and customize them depending on what you want.
