As you can see in the picture below, I have conditional notifications set up for one or multiple people to be able to receive when someone selects a particular school. We have utilized this for all of our past forms without any issues, but now, none of them are working for this new form.

Thanks for reaching out to Jotform Support. I have cloned your form and tested it on my end and the conditions are working as intended. Could you try to create a new Notification email and use it for your condition? Here's how to do it:
How to create a new Notification Email:
1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Click on Emails on the left side of the screen.
3. Click on Add an email.
4. Click on the Notification email.
5. Click Save.
How to update the Change Email Recipient condition:
1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Click on Conditions on the left side of the screen.
3. Hover your cursor over the Change Email Recipient condition and click on the Pencil icon.
4. Update the selected Notification email for Send.
5. Click on Save.
Also, you might want to check out on how to setup an SMTP to send out emails from your server. Here's how to do it:
- In Account Setting, click on Add Sender Email.
- Fill up the fields using your SMTP credentials.
- Click on Test Email to check if everything is working.
- Click on Save Email.
Once you are done setting up a Sender Email, you can then proceed to use it for your Notification/Autoresponder email. Here's how to do it:
- In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
- Click on Emails on the left side of the screen.
- Hover your cursor over the Notification email and click on the Pencil icon.
- Go to Advanced and scroll down to Sender Email.
- Select your custom Sender Email and click on Save.
