I’m looking for a way to automate appointment reminders a week out and a day before our customer’s scheduled appointment. Is this possible to set up with Jotform?

Hi Micah,
Thanks for reaching out to Jotform Support. Yes, it's possible to send an email reminder on a specific date automatically. Let me show you an example:
- In Form Builder, add three Date Picker elements and add a label.
- Next, create an Update/Calculate Field condition to populate a date to the two Date Picker fields. Click on Settings at the top.
- Select Conditions on the left side.
- Then select Update/Calculate Field.
- Create the condition below:
- Here are the screenshots of the conditions:
Checkout the screencast below to see that it populated a date 1 day before and 7 days before the appointment:
Next, you need to create two Autoresponder Emails you want to send on those dates:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- In the menu on the left side of the page, click on Emails.
- Now, click on Add an Email.
- Then, select Autoresponder Email.
- Edit the Email content.
- Under the Advanced tab, select the Date field that has a condition that populates a date 7 days before in the Send Email Later dropdown.
- Repeat the step above for the second email.
You can clone my demo form. We have a guide on How to Clone an Existing Form From a URL.
Give it a try and let us know if you need any other help.

Hi Micah,
In addition to the steps above, you can hide the two Date fields so your form responders won't see it. Here's how t do that:
- In Form Builder, select the Date field and right-click.
- Then select Hide.
Let us know if you have any other questions.
