    Hello!! I need some assistance choosing the correct Widget for my assessment form.

    I am assessing people's posture and workstation setup.

    I want to select a problem and then have a pre-written solution that populates onto the PDF for them to see.

    For Example, I select "Mouse too far away" and then it would populate with "Move mouse closer so that it can be used with elbow at side and wrist neutral."

    I have all the pre written problems and solutions, just not sure how to integrate those.

    I would want a problem section called "Chair", "Keyboard and Mouse", "Monitors", etc

    Hi Alicia,

    Thanks for reaching out to Jotform Support. If I understand your concern correctly, you would like to use a widget that sets an answer based on the user's answer and populates it into a PDF. If yes, then you can easily do that by adding a Conditional Logic in your form. Let me show you how:

    1. In your Form Builder, click on the Add Form Element located on the left side of your page.
    2. Under the Form Elements, go to the Basic tab and add a Single Choice element to your form.

    Next, we need to add a Long Text Field that will set the answer based on the selected option in the question:

    1. In your Form Builder, click on the Add Form Element located on the left side of your page.
    2. Under the Form Elements, go to the Basic tab and add a Long Text Field to your form.
    3. Click on the Gear Icon, and go to the Advanced tab in the Dialog that shows on the right side of your page.

    Now, let's add a Conditional Logic that will set the answer based on user selection:

    1. In your Form Builder, click on the Settings tab in the orange navigation bar at the top of the page. 
    2. Click on the Conditions tab on the left side of the page, and click on + Add Condition.
    3. Select the Update/Calculate Field and enter the following details:
    4. IF: Your question
    5. STATE: Is Equal To
    6. VALUE: your option
    7. DO: Calculate Field Value
    8. SUMMARY: Your set answer

    Now, let's attach a PDF in your form after user submission:

    1. In your Form Builder, click on the Settings tab in the orange navigation bar at the top of the page. 
    2. Click on the Emails tab on the left side of the page, and click the + Add Email button.
    3. Select the Autoresponder Email and do the following:
    4. Go to the Advanced tab and toggle the Attach PDF as On.
    5. Select the Default Document by ticking the checkbox beside it.

    To customize your PDF:

    1. Click on the arrow icon beside the Form Builder.
    2. Under the selection, click on the PDF Editor. You will then be redirected to the PDF Builder page.Best Widget Image 5 Screenshot 124

    In your PDF Builder, you can click the Add Element on your left side to add more fields or click the Paint Roller icon on the right side of your page to customize your document:

    That's it! Test your form, and you should receive a PDF with the populated answer after submission/ Check out my screencast below for the result:

    This is great! I have one question....what if there are multiple issues
    and I need to select more than one item? Lots of times, there's a few
    problems with their chair or their keyboard/mouse setup. Can I do the same
    conditional logic with the multiple answer?

    So this is in direct reply to your earlier answer showing me how to do a single choice question and add the conditional logic to generate a reply in a long text form on a PDF.

    However...many times, when I am looking at someone's setup, I will need to check more than one box in the answer. The mouse may be too far away, plus it may be too small, plus they may have tissue compression on the edge of the desk.

    So under Mouse Problems, I will need to check 3 boxes. And I would like 3 things to generate onto the PDF.

    Move mouse closer

    Get right sized mouse

    Gel pad or elevate hand to not compress tissue

    Right now as I've set it up, I can only choose one answer and then get one reply.

    Hi Alicia,

    Thanks for getting back to us. If these options were selected by the user, they will be included in the PDF Document. I checked your form and I don't see these options and answers. Have you not added them yet? Please add them then do a test submission and see if these questions show on the PDF Document.
