I have a series of company policies to initial and then sign when we onboard a new employee. Can I create that form here by uploading the existing PDF?

Hi Joe,
Thanks for reaching out to Jotform Support. Yes, this is possible. You can either convert the PDF to a Smart PDF Form or create a Jotform Sign Document from it. Let me show you how:
- Smart PDF Forms
1. On your My Forms page, click the Create Form button.
2. Select Import Form then the Import PDF form.
3. Click the Upload Document and select the PDF that you want to convert into a form. You can check out this guide on How to Import Your Own PDF With Smart PDF Forms.4. Once the Smart PDF Form is created, the system will try its best to detect the fields on the uploaded PDF and generate corresponding fields on the form. If the fields on the form are not mapped on the Original PDF, you will have to manually map them out. - Jotform Sign Document
1. Go to your My Documents page and click on Create Sign Document.
2. Now, you can upload your own document, or you can use the Demo Document to test it out.
3. Then, you can add more documents, or you can just continue by clicking on the Create Signable Document button.
4. That will redirect you to the Sign Builder page.- After creating your document, you can let the system detect fields automatically by clicking on the Detect Fields button, or you can manually match your fields with elements by dismissing the pop-up message:
