Our database manager needs the responses populated into an excel spreadsheet. Is this possible?

Hello, I currently work for a Literacy Service program. We give all of our students an exit survey which is currently on paper. I am working on putting the survey online (see form). Our database manager needs the responses populated into an excel spreadsheet. Is this possible?
